Requirements for Examination

At the time of application, all applicants must be able to provide anesthesia services independently and meet the following criteria:

1) Provide evidence acceptable to the Board of having graduated from a dental school

  • accredited by the Commission on Dental Accreditation of the American Dental Association, or
  • accredited by the Commission on Dental Accreditation of Canada, or
  • recognized by the World Health Organization and deemed equivalent to the above by the ADBA Board of Directors.

   A copy of the diploma or a letter attesting to graduation by the institution is acceptable documentation.

2) Provide evidence acceptable to the Board of a license to practice dentistry in the United States or Canada. The following is required:

  • A copy of the current licensure verification card, OR
  • A letter from the licensing board in the jurisdiction of licensure, are acceptable forms of documentation, AND
  • Documentation of any license restrictions to determine acceptability for examination.

3) Provide evidence of having completed an accredited Dental Anesthesiology Residency program of at least 24 months duration within the past 6 years prior to receipt of application by the Board. The following is required:

  • A copy of the Certificate of Completion from the dental school or accredited institution where the residency was completed, OR
  • A letter from the Anesthesiology Residency Program Director with the institutional seal embossed stating that such a residency was completed.

4) Provide a letter from the Anesthesiology Residency Program Director with the institutional seal embossed stating that the resident has gained the clinical competence necessary to independently provide the full scope of anesthesiology services for dentistry.

5) Provide evidence acceptable to the Board of a valid permit to administer general anesthesia as a dentist in the United States or Canada. The following is required:

  • A copy of the current general anesthesia permit, OR
  • A letter from the licensing board in the jurisdiction of licensure stating that a valid permit has been issued, AND
  • Documentation of any permit restrictions to determine acceptability for examination.

6) Provide copies of current completion cards from the American Heart Association or American Red Cross sponsored Basic Life Support – Health Care Provider Level and Advanced Cardiac Life Support courses.

Initial Application for Written Examination While in Residency

It is acceptable for the written examination to be taken within 6 months of anticipated residency completion only if:

  1. A letter from the residency program director is submitted stating that the applicant:
    a. has achieved an appropriate clinical competence level to take the examination; AND
    b. has progressed adequately in residency such that on-time graduation is anticipated; AND
  2. The applicant fulfills all other Entrance Requirements as outlined in Sections 1, 2 & 6.
  3. The applicant fulfills all other criteria prior to application for the oral examination

Initial Application More Than 6 Years From Residency

If residency training was completed more than 6 years before receipt of application by the Board, the applicant must reestablish his/her qualifications for examination. The following may reestablish qualifications for examination:

a. Complete a minimum of 6 months of continuous training in an anesthesia residency program acceptable to the Board. This training must include a significant component of anesthesia for dentistry, including nasal intubations and anesthesia for pediatric patients six year and under. In addition, a letter from the Anesthesiology Residency Program Director with the institutional seal embossed stating that the resident has gained the clinical competence necessary to independently provide the full scope of anesthesiology services for dentistry must be provided; OR

b. Document continuous practice of anesthesiology, including a significant portion of cases demonstrating intubated and/or non-intubated general anesthesia for adult patients and pediatric patients (defined as six years and under) and involving anesthesia for dentistry, of at least two days per week and/or 200 cases per year in each of the previous two years may qualify for Board examination through satisfactory completion of an entry examination acceptable to the Board. This may include both written and oral examinations, including presentation of anesthetic cases completed in the previous year as requested by the Board.
